MARSHALL ORTHOPAEDICS  -  GSMC / MARSHALL -  SNF ADMIT ORDERS>

 

1.  Admit to SNF :    Dr.

2.  Diagnosis :  

3.  Allergies:

4.  Resusciatation status : resuscitate

5.  Rehab potential : 

6.  Diet :

7.  Routine vital signs and intake/output; routine skin assessment

8.  Replace foley every 14 days, if foley cath has been inserted

9.  Physical therapy : evaluate and treat

                weight bearing status :

10.  Occupational therapy : evaluate and treat

11.  IV fluids: saline lock /flush per protocol

12.  Tylenol 650 mg po q4h prn headache or T over 100 degrees F

13.  Hydrocodone 5 mg po q4h prn mild or moderate or severe pain

                If allergic to codeine, give Darvocet N-100 tab 1 po q4h instead

14.  Surfak 240 mg po bid

15.  Milk of magnesia 30 mL po prn constipation

15a. Phenergan 25 mg po q6h prn nausea; if nausea persists despite phenergan, call internist

16.  Ambien 10 mg po at bedtime prn sleep

17.  Maalox 30 mL po q4h prn indigestion; if no relief, ok for Dulcolax supp 1 per rectum daily prn or Fleet enema 1 per rectum daily prn

18.  For infra-mammary or groin rash :  Lotrisone cream daily prn until healed

19.  Lovenox :   

20.  Any other medications as per attached orders

21.  Knee immobilizer to  knee when oob; ok to stop use when pt. has a strong

                straight leg raise

22.  Elevate  on   ; for knees, put pillow

under calf, not under knee

23.  Precautions :  

24.  Activity level : 

25.  Notify hospitalist of SNF admission...ask hospitalist please write for iv equivalent of p.o. home meds.

26. OK to renew medications as they expire.

27. Overhead frame and trapeze

28. CBC on second day after SNF admission, and every 3 days thereafter until discharge.
